Abstract
Measurement of a D-dimer level can be a powerful tool in the diagnosis of venous thromboembolism in the right context and with the right application. Used correctly, a clinical strategy combining pretest probability estimation and D-dimer measurement can allow safe discharge of patients with suspected pulmonary embolism without superfluous investigation or unnecessary interim treatment. We review clinical decision making strategies in the diagnosis of pulmonary embolism and propose means to minimise diagnostic error that might arise from knowledge of the D-dimer result prior to clinical assessment.
