Abstract
The National Health Service (NHS) Clinical Outcomes Framework describes five domains of health care that, from April of this year, provide a structure against which NHS performance can be judged, against which commissioners of health care in England, responsible for the effective spend of approximately £95bn of public money, can be held to account, and through which quality improvement throughout the NHS can be driven [1]. The first domain relates to preventing people from dying prematurely. Following the publication in February this year of the Francis Report into the failures, including high mortality, at Mid Staffordshire NHS Foundation Trust [2], Professor Sir Bruce Keogh conducted a review into the quality of care and treatment provided by hospital trusts with persistently high mortality rates [3]. The 14 trusts assessed had been outliers for the last two consecutive years on either the Summary Hospital-Level Mortality Index (SHMI) or the Hospital Standardized Mortality Ratio (HSMR).
