Education and follow-up cut HF readmissions.

Abstract
A heart failure pilot project at Indiana University Health Ball Memorial Hospital in Muncie cut the readmission rate by 50%. Patients receive intensive coaching while in the hospital, and they receive weekly follow-up calls after discha rge. Team members use the teach-back method to ensure patients understand their treatment plan. They teach patients to use the Heart Failure Zones to determine what to do if their conditions change.
